Использование тиазидных диуретиков обычно ассоциируется с потерей электролитов, главным образом калия, а также натрия, хлора, бикарбоната и магния. Потеря других электролитов, таких как фосфат, бромид и иодид является, как правило, незначительные. Истощение калия и магния может привести к сердечной аритмии и остановки сердца. При других электролитных осложнений включают метаболический алкалоз и гипонатриемии, которая редко угрожает жизни. Терапии с тиазидными диуретиками следует вводить с осторожностью в больных или предрасположенных к жидкости и электролитов истощения, в том числе пациентов с первичным и вторичным альдостеронизмом (могут иметь низкие уровни калия); людям с тяжелой или длительной диареей или рвотой; и те, у кого плохое состояние питания. Жидкости и электролитных нарушений должны быть исправлены до начала терапии, и артериального давления, а также концентрации в сыворотке электролит периодически контролируется и поддерживается в пределах нормы во время терапии. Пациентов следует предупредить о необходимости немедленно сообщать знаки и симптомы жидкости и электролитов дисбаланс, в том числе сухость во рту, жажда, слабость, летаргии, сонливость, беспокойства, мышечных болей или судороги, мышечной усталости, гипотонии, олигурия, тахикардия, аритмия, или желудочно-кишечные заболевания, такие как тошнота и рвота. Оцифрованный пациентов и пациентов с наличием в анамнезе желудочковой аритмии следует проводить тщательный мониторинг, поскольку развитие гипокалиемии может быть особенно опасна у этих пациентов. Риск гипокалиемии может быть сведено к минимуму медленным диуреза, снижение тиазидов дозы, прием препаратов калия или комбинация с калий-сберегающих диуретиков.
