Снижение эффективности и возможной задержкой выведения из петлевых диуретиков может возникать у пациентов с тяжелой почечной дисфункции. Эти лица могут требовать высоких дозировках, которые связаны с повышенным риском развития электролитных нарушений (гипонатриемия, гипохлоремический алкалоз, гипокалиемия, гипомагниемия, гипокальциемия) и ототоксичность (шум в ушах, потеря слуха). Терапия петлевые диуретики следует назначать с осторожностью у пациентов со значительно нарушенной функцией почек. Продление интервалов дозирования может быть целесообразным для предотвращения кумуляции препарата. Больной должен тщательно следить за знаки и симптомы жидкости и электролитов дисбаланс, в том числе сухость во рту, жажда, слабость, летаргии, сонливость, беспокойства, мышечных болей или судороги, мышечной усталости, гипотонии, олигурия, тахикардия, аритмия, или желудочно-кишечные заболевания, такие как тошнота и рвота. Чрезмерный диурез следует избегать, поскольку это может вызвать обезвоживание и гиповолемия, которая может привести к резкому снижению клубочковой фильтрации и почечного кровотока. При увеличении азотемии и олигурии возникают во время лечения тяжелой прогрессирующей почечной недостаточности, или если функция почек становится все хуже и хуже, как указано на рост Бун или креатинина в сыворотке крови, следует рассмотреть возможность прерывания или отмены терапии.
