Опиатных агонистов может производить значительные центральной нервной системы и угнетение дыхания различной длительности, особенно при назначении в высоких дозах и/или при быстром внутривенном введении. Апноэ может быть результатом снизилась активность дыхательного центра, а также повышенное сопротивление дыхательных путей, и ригидность дыхательных мышц может произойти во время быстрого внутривенного или когда эти агенты используются при индукции анестезии. В терапевтических дозах анальгетик, дыхательной эффекты обычно не имеет клинического значения, за исключением пациентов с предсуществующей легочной недостаточности. Терапии с опиоидными агонистами следует избегать или назначать с особой осторожностью и начато в низких дозах у пациентов с тяжелым поражением ЦНС депрессии; апноэ сна; гипоксия, гипоксия или гиперкапния; обструкция верхних дыхательных путей; хроническая легочная недостаточность; общества с вентиляторными резерва; или другими респираторными заболеваниями. При наличии избыточной секреции дыхательных путей, применение опиоидных агонистов может также быть проблематичным, поскольку они снижают активность цилиарного и уменьшить кашлевой рефлекс. Осторожность также рекомендуется в пациентов, которые могут быть на увеличенном риске для респираторной депрессии, например, коматозных пациентов или тех, с травмой головы, внутричерепная поражения или внутричерепной гипертензии. Рекомендуется клинический мониторинг функции легких, и оборудование для реанимации должны быть немедленно доступны, если используется парентеральное или нейроаксиальной маршруты. Налоксон может быть введен для обратного клинически значимого угнетения дыхания, которое может быть продлено в зависимости от опиоидных агентов, накопленной дозы и пути введения.
