Трициклические и тетрациклические антидепрессанты (ТЦА) обладают антихолинергической активностью, к которым пожилые пациенты особенно чувствительны. Третичные Амины, такие как амитриптилин и тримипрамин, как правило, демонстрируют большую антихолинергических эффектов, чем другие агенты в классе. Терапия ТЦА следует назначать с осторожностью в пациентах с существовавшие ранее условия, которые могут быть усугублены антихолинергического действия, такие как непроходимость или задержка мочи; закрытоугольная глаукома, внутриглазная гипертензия неочищенных или неконтролируемой первичной открытоугольной глаукомы, а также желудочно-кишечные обструктивные нарушения. У пациентов с закрытоугольной глаукомой, даже средние дозы могут вызвать приступ. Глаукому следует рассматривать и под контролем до начала терапии ТЦА, и внутриглазное давление контролируется во время лечения.
